Pay Rate:  $22.19 - $35.50/hr.

 

*** This position would be Part Time (10 hrs. per week/20 hrs. per pay) working in the Clinic from 8:00am - 1:00pm every Saturday and Sunday.

 

 

SUMMARY OF JOB RESPONSIBILITIES:

To assist Clinic Manager in maintaining high quality services to the residents and staff served in the Laurel Lake Wellness Center. Create and maintain clinical records for those utilizing the Wellness Center. The Wellness Nurse will demonstrate behavior consistent with the Mission Statement and Core Values.

 

QUALIFICATIONS

 

EDUCATION:

Educational Requirements:

  • Graduate of Licensed Practical School of Nursing or
  • Registered Nurse diploma/degree
  • Healthcare Provider CPR/First Aid Certified

Licensure, Certification and Registration:

  • LPN current license in state of Ohio
  • RN current license in state of Ohio

 

EXPERIENCE:

Work Orientation and Experience:

  • Experience with long term care, home health, geriatrics
  • Managerial Skills

Other Requirements Specific to Assigned Area:

  • High level of patience

PRINCIPAL JOB ACCOUNTABILITIES:

  • Meets productively standards; complete work in a timely manner.
  • Follow up emergency response reports, respond to emergencies in independent living, encourage updating Col Paks upon discharges from Crown Center or hospital, schedule routine labs, supervise and assist with “drop-in” visits, facilitate resident transfers and admissions to Crown Center, administer and read Mantoux tests for staff.
  • Assist clinic staff with other assigned duties
  • Perform clinical functions with -in the scope of practice                               

Assist with the morning medication pass for assisted living routinely on weekends.

  • Cultures, collecting specimens, wound dressing changes, irrigations, etc.
  • Schedule appointments, labs, x-rays and other test, call Residents with result
